After scheduling your appointment, please review the appointment confirmation email and follow the additional instructions to watch the ‘How To Inject‘ videos.
- Be sure to completely fill out the forms! Like, we really do need your first & last name.
- Omitting info or required fields will cause delays.
- You must upload a photo of your ID or drivers license within the Intake process.
- After you submit your intake and consent forms, click the Scheduling button to head over to our online scheduler.

- Appointment scheduled? Be sure to review your appointment confirmation email. It contains info on how to connect with the physician for your video chat.
